What is Cauda Equina Syndrome?

Cauda Equina Syndrome (CES) is a rare condition where the bundle of nerves at the bottom of the spinal cord — the cauda equina — gets severely compressed. Those nerves control the legs, the bladder, the bowels and sexual function, so when they stop working it causes symptoms like leg weakness, numbness around the saddle area, and loss of bladder or bowel control. It is treated as a medical emergency.

How common is CES?

It's rare — roughly three people in every 100,000 are affected each year. It can happen at any age, but it's most often seen in adults. Because it's uncommon, it's often missed in early consultations — which is exactly why awareness matters so much.

What causes CES?

The most common cause is a slipped disc in the lower back pressing hard against the nerves. Less common causes include spinal stenosis (a narrowed spinal canal), tumours, infections such as an epidural abscess, fractures or trauma, and rarely, complications after spinal surgery or an epidural.

How is CES diagnosed?

Diagnosis is made urgently with an MRI scan of the lower spine, alongside a neurological examination — checking leg strength, reflexes and sensation. A doctor may also do a quick rectal examination to assess the saddle-area nerves. If CES is suspected, the MRI should not be delayed.

The main treatment is emergency decompression surgery to take the pressure off the nerves. Surgeons aim to operate as soon as safely possible — ideally within 48 hours of symptoms starting. If an infection caused the compression, antibiotics are given as well. If a tumour was the cause, further cancer treatment may follow.

Can you fully recover from CES?

Recovery varies. Some people recover completely, especially when surgery happens early. Others are left with longer-term effects — bladder or bowel problems, sexual dysfunction, ongoing pain, or weakness in the legs.
